Educational Loan Repayment for Health Care Professionals

The Vermont Educational Loan Repayment Program for Health Care Professionals is funded by federal and state funds, and is administered by the UVM Larner College of Medicine Office of Primary Care and Area Health Education Centers (AHEC) Program.

The purpose of this loan repayment program is to respond to the workforce shortage of certain health professionals in Vermont, thereby increasing access to health care for underserved populations and communities. This program aims to contribute to a stabilized workforce at the specific work site (as identified in application materials) and timeframe.

Important note:  Program eligibility and selection criteria can change annually.  Please refer to the program overview and participant eligibility for this cycle's information.

2018 Applications Deadline: Thursday, September 14, 2017 (received by the close of business)

The 2018 application has two parts: 1) CORE application and 2) Employer Section E.

  • Both parts must be completed for the application to be complete
  • Only complete applications will be considered
  • Applications must be typed and received by September 14, 2017
  • It is the applicant's responsibility to ensure that the application has been received and is complete by the stated deadline

Use Internet Explorer as your browser.

Save the PDF applications to your computer
     To save, open the document.  Go to File, "save as" -- select location on your computer
     Rename the CORE file.  We suggest "2018 PC ELR your last name, your first initial"
     Rename the EMPLOYER SECTION E file.  We suggest "Section E your last name, your first initial"

Email section E to your employer for completion.  Section E can be returned by your employer directly to AHEC by September 14, 2017.  It is the applicant's responsibility to ensure that Section E is completed and sent to AHEC by the stated deadline.

Complete the forms using the fillable PDF (i.e., must be typed) then print, sign, and mail to AHEC.
