The project works with health care practitioners to improve outcomes for children and youth with chronic illness. Current efforts focus on the population of children cared for in The University of Vermont Children’s Hospital Specialty Center, in particular those children with diabetes, cystic fibrosis, kidney disease, inflammatory bowel disease (IBD). In addition to clinic specific projects, we work collectively to improve the care of children with chronic conditions.
Our recent focus has turned to transitions of care, and we are engaging specialty clinics from the UVM Children's Hospital Specialty Center. Through this work, we aim to provide a framework for the safe and appropriate transition of care from pediatric specialty care to adult primary or specialty care.
