Center on Aging Geriatric Fellowship

Policy Statement
The focus of this one-year fellowship is to participate in the ongoing development and implementation of Support and Services at Home (SASH). SASH is a key component of Vermont’s health care reform initiative – the Blueprint for Health – a medical home model. SASH provides community-based practice support through one streamlined collaboration of community providers, all serving on the SASH team. SASH provides care coordination, transitions planning and self-management education to Medicare eligible individuals living in the community. The fellowship comes with a stipend.
For one year prior to beginning Advanced Integration, fellows are exposed to an enriching first-hand experience in gerontology. The final level of medical studies is deferred, while the student is on a leave of absence, and the fellow joins the next
graduating class. Former fellows have found the experience very rewarding.
