The Center on Aging at The University of Vermont is pleased to present this program whose goal is to provide current, research-based information on topics relevant to both family and professional caregivers who work with our rapidly growing population
of older adults. For the second year, the conference will offer two different tracks:
- Family Caregiver Track
- Professional Track
Participants will come together throughout the day, yet there will be separate "breakout" sessions for family caregivers and professionals. There will be an opportunity to interact with colleagues from numerous Vermont organizations, including exhibitors.

Dr. Karen Wyatt is the bestselling author of the book, What Really Matters: 7 Lessons for Living from the Stories of the Dying. Dr. Wyatt’s book contains stories of patients she cared for as a hospice doctor and the spiritual
lessons she learned from them at the end of their lives.
Dr. Wyatt also hosts End-of-Life University, an online interview series that features conversations with experts who work in all aspects of end-of-life care.
She is widely regarded as a thought-leader in the effort to transform the way we care for our dying in the U.S. In addition, she is valued for her application of spiritual principles to illness and healthcare, and teaches that in order to live
life fully we must each overcome our fear of death and embrace the difficulties that life brings us.

This conference is designed to enable participants to gain knowledge pertaining to a key areas of caregiving for older adults: End of Life Care and Transitions. The symposium also seeks to strengthen the connections between family caregivers and
professionals. In 2019, topics will include:

- Advance Directives and Advance Care Planning
- Complex decision making when multiple agents are involved
- Grief and Bereavement, as experienced by families and professionals
- End of Life Doulas, an additional resource
- Hospice and Those Living with Dementia
- Hospice and Palliative Care services

Registration Fees
- Professionals: $35
- Family Caregivers: $20 (discounted registration is intended for those caring for family members)
- Continuing Education credit: add $20
Accreditation

In support of improving patient care, The Robert Larner College of Medicine at the University of Vermont is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education
(ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
This program has been reviewed and is acceptable for up to 6.0 nursing contact hours.
The Vermont Office of Professional Regulation has approved this course for 6.0 hours of continuing education for Allied Mental Health Practitioners.
The Vermont Office of Professional Regulation has approved this course for 6.0 continuing education hours for Nursing Home Administrations.
As a Jointly Accredited Organization, The Robert Larner, M.D. College of Medicine at the University of Vermont is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education
(ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit.
The Robert Larner, M.D. College of Medicine at the University of Vermont maintains responsibility for this course. Social workers completing this course receive 6.0 continuing education credits.
