Improving Prenatal Care in Vermont: A Practice Level Toolkit and State Guide

The Improving Prenatal Care in Vermont (IPCV) Toolkit and State Guide are sets of materials and tools designed and tested in the Improving Prenatal Care in Vermont Learning Collaborative, a 15-month quality improvement initiative. The state-wide initiative was designed to help improve pregnancy outcomes of low weight and preterm birthrates by implementing updated, evidence-based prenatal care, and developing improved office systems.  As a result of this innovative work, a toolkit was compiled to assist prenatal care providers in their commitment to optimum prenatal care delivery.  This toolkit is now available to you.

The Toolkit provides you with practical advice on how to easily implement tools, track improvements and build needed partnerships with existing state and community resources. The tools and resources are intended to accomplish the following: 

  • Identify evidence based ‘Best Practice’ guidelines for prenatal care in nine topic areas (listed at right);
  • Recommend screening tools for integration into your practice workflow or for use as a reference while updating currently used tools;
  • Incorporate innovative approaches to implementing change in how prenatal care is delivered - assessment, intervention and follow-up with suggested methods of tracking changes to ensure they are actual improvements;
  • Incorporate techniques for streamlining referral processes and making connections to community support agencies.
The purpose of the State Guide is to provide information for implementation of a similar state-wide initiative.
