Bridging the Divide: Primary Care and Public Health Conference

Overview

For the past seven years the Bridging the Divide conference has focused on enhancing collaboration between Primary Care and other disciplines such as Mental Health, Pharmacy, and Oral Health. This year we explore the myriad connections between Primary Care and Public Health. As our health system moves from a fee-for-service payment model to one focused more on value-based payment and overall population health, the linkages between Primary Care and Public Health have never been more important. This paradigm shift is being facilitated by an increasing availability of population health data from various sources, and a general desire to better align, coordinate and integrate public health and patient care to improve population health outcomes.

The goal of this conference is to bring together health care and public health professionals to learn more about their respective points of view and priorities. We are pleased to present keynote speaker John Auerbach, the Associate Director for Policy and the Acting Director of Office for State, Tribal, Local and Territorial Support of the Centers for Disease Control and Prevention (CDC). We will also offer sessions regarding the role for community health workers in our evolving health system, health promotion and illness prevention, and a variety of workshops with a primary care/public health focus. In the early afternoon we will have a “speed” meeting session in which participants will explore various functions of the Vermont Department of Health.

Learning Objectives

  1. Describe the Centers for Disease Control and Prevention’spublic health priorities, including the 6/18 Initiative: Accelerating  Evidence into Action.
  2. Describe potential roles community health workers can play in Vermont’s health system.
  3. Identify the role of health promotion to improve population health.
  4. Discuss the intersection of primary care, dental and public health, providing specific examples.

Accreditation

The University of Vermont, College of Medicine is accredited by the American Nurses credentialing Center (ANCC), the Accreditation Council for Pharmacy Education (ACPE), and the Accreditation Council for Continuing Medical Education (ACCME), to provide continuing medical education for the health care team.  

The University of Vermont designates this live activity for a maximum of 7 AMA PRA Category 1 CreditsTM.  Physicians should claim only credit commensurate with the extent of their participation in the activity.  

This program has been reviewed and is acceptable for up to 7 Nursing Contact Hours.  

Credit is pending for the following providers: Social Workers, Psychologists, and Mental Health Professionals.

This conference has been approved for 7 non-clinical credits for VT Dentists.

Conference Location

The conference will be held at The Essex in Essex Junction, Vermont, located at 70 Essex Way. A few rooms have been reserved at The Essex for conference participants.
