The Student Affairs Office works closely with the University's Medical Alumni Office, and we continue to serve College of Medicine graduates throughout their careers.
When applying for licensure or employment in a new state or institution, please complete our Medical Education Verification/Document Request Form and submit this along with and other forms you need completed. By law we can not forward such documents without your signed consent, so be sure to complete and sign our request form. Requests may be mailed or faxed, and you may e-mail a scanned copy of the signed request form; an e-mail request that doesn't include this signed form is not a valid request and will not be honored. When completing the request form, please type or clearly print all information on the form. We can not be held responsible for undeliverable documents if we can't read your handwriting. A Note About Requesting Transcripts:
If you are a graduate of the Class of 2003 or later you should request medical school transcripts through the UVM Registrar's Office (unless it is part of a licensing packet which we are completing, in which case, we will request the transcript on your behalf). Graduates from the Class of 2002 and earlier will still get transcripts from Student Affairs.
The Registrar's form is available online at the Registrar's Office website, as well as here, and hard copies are available in the office at 360 Waterman.
Replacement Diplomas:
If you need to request a replacement diploma, please complete the Diploma Reorder Form and return it to the Student Affairs Office along with the $50 replacement fee, payable to the University of Vermont. Lead time for replacement diplomas is approximately 6 weeks.
Requests to verify residencies or fellowships are not handled by the Student Affairs Office. Those requests should be forwarded to the Office of Graduate Medical Education at Fletcher Allen Health Care.

If you were not enrolled in the MD program at the College of Medicine you should not request documentation from the Student Affairs Office. Summer Anatomy and Pharmacology students from other schools are registered through the Graduate College and should contact the UVM Registrar's Office at 802-656-2045.
The Student Affairs Office does not verify laboratory assistant training or research. Please contact the concerned department directly.
